• Outpatient/ Inpatient required
• No admissions required
• Hospital/ Surgery center privileges required
• No temporary privileges available
• 0-1 patients per shift
• Hospital is stroke certified

Physician Requirements:

• Board Certified
• No license required to bid
• BLS required
• 60-90 days credentialing timeframe
• Trauma Level 3
• Support Staff: RN 2, MA 3, PA/NP 5

• Upper GI endoscopy
• --> with biopsy
• --> with foreign body removal
• --> with fulguration of bleeder (cautery vs. laser)
• --> With injection of varices
• --> With PEG (percutaneous endoscopic gastrostomy)
• Esophageal dilatation/manometry
• Small Bowel biopsy
• Lower GI endoscopy
• Sigmoidoscopy
• --> Flexible
• Colonoscopy
• --> with biopsy
• --> with excision of polyp
• Endoscopic ultrasound (EUS)
