Credentialing: 120 days
License: LA or IMLC
Other: Must be Board Certified, clean background, Provider will own their own panel of patients and follow them while they are there Need consistency of coverage. Call will be based on providers own patients. Open to someone to come in for 4 -10 hour days or 5-8 hour days An ideal rate would be 24 hour coverage with 8-10 hours patient care built in with call for patients they are seeing.
